Improvement in therapy against tabun poisoning due to combined pretreatment of oximes and atropine

Pyridinium oximes have been investigated for many years as compounds with a great potential in the treatment of organophosphorus compounds poisoning including insecticides and nerve agents. Although oximes are known primarily as reactivators of phosphorylated AChE, they are also reversible inhibitors of cholinesterases and in that way they act as protectors of AChE against phosphorylation. In this study an attempt was made to establish whether effect of pretreatment and therapy with new synthesized oximes K048 and K074 in conjuction with atropine have potency to provide in vivo protection of experimental animals against multiple LD50 doses of nerve agent tabun. TMB- 4, which is the best-known antidote in tabun poisoning, was used for comparison. Oxime doses of 25 % or 5 % of its LD50 were used for pretreatment 15 minutes before tabun-poisoning and for therapy one minute after tabun administration to mice. Combined pretreatment and therapy of K048 (25 % of its LD50) and atropine (10 mg/kg body weight) showed low acute toxicity (8002.0 µ ; ; g/kg) while TF and TD were 25.2 and 20.0, respectively. However, the best therapeutic effect was obtained when oxime TMB-4 (25 % of its LD50) in combination with atropine was used in both pretreatment and therapy. This regimen insured survival of all tested animals after the application of 25.2 LD50 doses of tabun. These positive effects could be explained by the synergistic effect of the applied combination.
