Impairment of spontaneous movements in Parkinson’s disease patients: movement-related evoked potentials study

Background: Movement-related evoked potentials (MREPs) represent an activity that emerges in brain motor areas about 1.5s before movement onset. MREPs reflect movement planning, preparation and execution. Spontaneous movement execution has shown to be impaired in Parkinson’s disease (PD), causing changes in MREPs. Objectives: The aim of the study was to examine diagnostic value of MREPs in PD. Considering the inconsistent results of previously performed studies, we realized the importance of choosing the group of PD patients as homogeneous as possible, considering patient’s age, disease severity, same affected side, dominant hand, medication and other factors that might influence MREPs. Methods: 22 participants were divided in two groups: 1. PD patients - by the UK PDS BB Criteria (9 males, 2 females ; all right side onset PD ; age 56.1±5.2 ; Hoehn and Yahr stage 1-1.5) and 2. healthy controls (10 males, 1 female ; age 23.5±0.7). They were asked to perform spontaneous thumb movements repeated every 5-10s. Electroencephalogram (EEG) was recorded and evoked potentials were extracted. Results: Compared to controls PD patients showed the later onset of the beginning of MREPs, known as the Bereitschaftspotential (BP). Statistically significant BP latency delay was observed for an affected hand. Conclusions: This study confirmed the influence of PD on MREPs. MREPs could be a useful tool for diagnosing PD, especially in early, presymptomatic phase, distinguishing the affected side of body, monitoring disease progression and efficacy of therapy. Further work is needed in establishing precise normative data for MREPs of PD patients and healthy controls.
