Prevention of diabetes complications and death by CroDiab registry

Objectives: The aim of this study was to create a model of chronic diabetes complication development and death assessed by glycaemic control based on diabetes registry. Also, we sought to evaluate the long-term effect of quality inclusion and follow-up of relevant CroDiab registry data on chronic complications and mortality by means of simulation. Study Design: Prospective cohort study was simulated using simulation modelling. We applied a simulation model designed to simulate changes in the frequency of diabetic complications and death of persons with diabetes over a 20-year period depending on the proportion of diabetic population registered. Methods: The research was carried out by analysing the CroDiab registry data and assessing simulation parameters according to published literature and obtained results. Simulation was performed on data of persons with type 2 diabetes in Croatia, registered by primary health care providers. The model was developed using STELLA program (v 9.0.2). Results: Simulation results showed that without intervention, 14.60% of the total baseline diabetic population would not have developed chronic complications or die over a span of 20 years. Without the influence of CroDiab surveillance, the number of persons with chronic complications would decrease by 36.01% while 60.73% of the baseline population would not survive. However, a gradual increase in patient coverage by CroDiab registry surveillance revealed an increasing number of diabetic patients without complications and a decreasing proportion of no survivals. Complete registration coverage would yield additional 2.24% survival in baseline patients over a period of 20 years. Conclusions: Continuous registration and surveillance via CroDiab registry reduces the rate of chronic complications and death in persons with diabetes. The effect increases with higher registry share, justifying efforts to increase surveillance coverage. With the development of technology, the potential of CroDiab exceeds the function of dynamic data integration platform, developing into an efficient tool in tackling diabetes epidemic on the primary health care level.
