Laparostomy and individualy planned re-explorations for patients with severe intraabdominal infections

We have treated 57 patients with severe intra-abdominal infections, 35 male and 22 female, using our own method. Laparostomy were coverd with suttureless plastic sheet combined with individually planned re-explorations. The main advantage of plastic sheet over the mesh is that it does not stick to the underlying structures, prevents heat and fluid losses. Average numbers of reexplorations were 5, minimum 2 and maximum 12. APACHE II score were 21 on average. The mortality resulted in 16/57 (28%) patients. Complication occured in 13/57 patients, but only 7 is closely related to applied treatment. Laparastomy closure after infections were eradicated were carried using anterior rectus muscle sheet, difficulties were observed only in patients with stoma positioned improperly.
