Pulsed dye laser in the treatment of nail psoriasis
Psoriasis is one of the diseases that most often involves the nails. Nail psoriasis is a significant cosmetic handicap, it interferes with patients job, and causes pain, and it is often refractory to traditional treatments which includes nail care and topical therapies. On the other side, more effective systemic therapies only for treatment of nail psoriasis is usually not justified. Materials and methods: Pulsed dye laser (PDL) is a valid end effective treatment option especially for patients with only nail psoriasis. We present a young female patient with psoriatic trachyonychia and nail pits, onycholysis, subungual hyperkeratosis, without psoriatic lesions on the skin. She was treated with pulsed dye laser (7 mm, 6 ms, 9 J/cm2) once a month , for six months. Results: Six PDL treatment resulted in improvment of both nail matrix and nail bed clinical parameters. Besides moderate pain during and two hours after the treatment there was no significant adverse events. Conclusions: Psoriatic lesions improve with PDL treatment by the obliteration of their supporting vasculature. Besides destruction of blood vessels, PDL can reduce the number of helper T cells in the dermis and cytotoxic T cells in the epidermis, and normalize epidermal proliferation , which improves the clinical signs of psoriasis vulgaris. PDL is a well tolerated, safe and effective treatment option for nail psoriasis.
