Eosinophil cationic protein (ECP) and airway resistance in asthmatic children

The Aim: was to determine the value of measured parameters within the group of healthy children (Control) and int he asthmatic groups under the various therapies: Inhaled steroid (IS: fluticason propionate: 100-200 µg daily), Specific immunotherapy (SIT) anad Sodium cromoglicate (SC: 60-80 µg daily) group. Subjects and Methods: In 161 childeren (96 boys and 65 girls) diagnosed as mild and moderate asthma and 40 healthy children (20 boys and 20 girls), we measured: Serum eosinphil cationic protein (ECP, Radioimmunoassay method), Airway resistance (RTot, measured with Masterlab Jaeger), Residual volume (RV) and Residual volume of Total lung capacity (RV%TLC), measured by single breath method with carbon monoxide on Masterlab Jaeger. In Statistic Analysis: the Oneway analysis of variance, Mann-Whitney and Chi-square tests were used. The Results: were as follows: Control IS SIT SC p No. of children (201) 40 86 48 25 Age (y) 11.1 ± 2.2 9.7 ± 3.2 10.8 ± 3.6 11.4 ± 3 Therapy (y) 0 1.6 ± 1.8 3.6 ± 1.6 2.3 ± 1.9 ECP (µg/L) 10.5 ± 8.4 23.9 ± 20.5 24.5 ± 24.7 10.2 ± 11.7 0.012 RTot (%) 149 ± 62 271 ± 245 266 ± 177 211 ± 101 0.14 RV (%) 150 ± 70 267 ± 127 247 ± 175 255 ± 143 0.001 RV%TLC (%) 139 ± 47 184 ± 40 180 ± 59 184 ± 52 0.002 Results reported as average ± standard deviation. ECP:Normal values <19.7 µg/L, RTot, RV and RV%TLC are expressed as percentage of predicted value according to Zapletal. There were no statistically significant differences of ECP values between Control and SC group, but the difference was significant between IS and SIT in relation to the Control group. Although the RTot values werw lower for Control group, there were no statistically significant differences of RTot values among the Control and other groups. IS, SIT and SC groups did not differ in parameters: RV and RV%TLC, while this difference was statistically significant in relation to the Control group. Conclusion: The best results in normalisation level of serum ECP was obtained in SC group. RTot level was not statistically significantly different among examined groups suggesting the equal therapeutic effect in children with mild and moderate asthma. Increased values of RV and RV%TLC in IS and SIT and SC groups in relation to the Control group suggest the incomplete remission and the need for more intensive treatment (by increasing of inhaled steroid dose).
