Adjuvant intraoperative photodxnamic therapy in experimental models

Although surgery is still the preferred method of cancer treatment, complete eradication may not always be achieved because of microscopic residua or infiltratio of important strictures. Photodynamic therapy (PDT) may be used as adjuvant treatment in order to eradicate macroscopically invisible tumour after surgery. Since the limitation of this treatment is the depth of light penetration, combination with surgery is particularly important. The aim of this research was to prove the model of intraoperative adjuvant PDT as a helpful method of achieving complete tumour eradicatio. 10e6 cells of CMC-1 fibrosarcoma were injected into the thighs of CBA/HZgr mice. Photofrin II (2, 10, 15 or 20 mg/kg) was given with 250 mW/square cm of 630 nm wavelength light from a non-laser cold-light source for 20 min. One group of mice was treated when tumour volumes were about 120 ccm and the other group received treatment when tumour volumes were about 500 ccm. Cure rates of 120 ccm tumours increased with opening the skin over the tumour. 57% of mice with 120 ccm tumours were cured using PDT (10 mg/kg) after the skin over the tumour was opened, compared to 20 % using only PDT (10 mg/kg). No mice with 500 ccm tumours were cured by surgery or PDT alone (20 mg/kg), but the combination of surgery and PDT (20mg/kg) cured 71% of mice (p<0.05). In this study, cure rates were improved by opening the skin over the tumour and complete eradication of large tumours was significantly improved by using adjuvant intraoperative PDT. Based on these results and our former research in PDT, we believe that PDT may play an important role in clinical oncology as a valuable addition to the standard surgical treatment of tumours.
