Biomechanical Changes in the Lumbosacral Region

The problem of surgical approach to the spinal canal, especially in operation of the intervertebral disc, has not yet been solved. In the literature we find different and sometimes even contradictory opinions concerning interhemilaminectomy, hemilaminectomy or complete laminectomy. This paper presents our worked where we tried by strain analysis to determine the loading changes after removing some parts of the vertebral arch. The results showed that the strain quantity changes very little in vertical and lateral loading after the removal of the upper or lower part of the vertebral arch (model of interhemilaminectomy). Based on the results from mechanical tests and qualitative observations, we come to the conclusion that the most convenient approach to the canal is interhemilaminectomy. Hemilaminectomy and complete laminectomy equally disturb the biomechanics of the lumbar part of the spine.
