CRO PharmaDiab Programme – A part of the Croatian National Programme for the Protection of Diabetes Mellitus

Croatia has population of 4.8 millions and 116.295 diabetics, with an overage incidence of 2.35%. The “Croatian model” of organisation of health care for diabetic patients was included in the St. Vincent Declaration in 1989, and Croatian pharmacists have been included in the current National Programme for the Protection of Diabetes Mellitus, since November 1997. Some important steps preceded the implementation of CRO PharmaDiab Programme at national level. The parallel holding of Fourth meeting for the implementation of the St. Vincent Declaration and EuroPharm Forum/WHO training seminar on “Diabetes care” (Lisbon, February 1997), as well as joint activity, the setting up of a pharmacist working group during the 4the St. Vincent Declaration, was of the crucial importance for the implementation of our programme. The CRO PharmaDiab programme was prepared following the directions of the EFPA project “Diabetes care”, and in accordance with the National Program requirements. The proposal CRO PharmaDiab programme was well accepted, especially by the patient group. So far, we have implemented the Phase I of our programme, which deals with updating pharmacist’s knowledge about diabetes and its adequate pharmacotherapy. For this purpose, we organised two-day courses in collaboration with the Vuk Vrhovac Clinic for Diabetes, Endocrinology and Metabolic Diseases (Country and Regional Centres for Diabetes), a scientific and educational institution and WHO Collaborating Centre. About 120 pharmacists from all over the country have been included in the project and attended these courses. The courses comprised the following lectures: Aims of the CRO PharmaDiab programme, What does it mean to be a diabetic?, Questions and needs of diabetics in the pharmacy, How to treat the illness?, Why, how and how much to eat?, What can replace sugar?, How to keep the disease under the control?, Acute and chronic complications of diabetes, etc. The courses also included eight workshops with practical work (Different kinds of insulin syringes, pens and the use of different insulin products and mixtures, Tools for glucose measurement, Nutritional dietetic products, etc.). The courses were organised according to modern educational standards. The active pharmacist knowledge acquired on these courses was evaluated. The evaluation was also performed with the aim of examination of pharmacist interest in the field of diabetes care and their will in taking an active role in the patient counselling. The results of evaluation showed a great interest, pharmacist satisfaction with the organisation of courses, and the need for further education. Two similar courses are foreseen in the near future. The list of pharmacies in which patients can obtain information about diabetes is cited in the “Izvor”, the official Journal of Croatian Diabetic Association. We would also like to mention that two students’ degree theses have been made in the field of Pharmaceutical care–Diabetes care. General statements that comprising the role of Croatian pharmacist in the diabetes care, entitled The Hvar Declaration, were accepted in May 1998.
