Intracranial arachnoid cyst of uncommon site

Arachnoid cysts are benign developmetal cysts that occur in the cerebrospinal axis in the relation to the arachnoid membrane. Using CT and MRI increasing numbers of of this cysts are recognized. The mechanism of cyst formation is not completely undrestood. The most common site is in sylvan fissure &#8211 ; more than fifty percent, and the rarest site is intraventricular and intradiploic site..Our analysis was made in last five years dring which we have operated 12 arachnoid cysts of uncommon site. They were: 5 suprasellars, 4 quadigeminal plate, 1 intraventricular , 1 sellar and 1 clial cyst. They were mostly presented in infant population, and only one patient was middle age (sellar cyst). The main clinical manifestations were due to the obstructive hydrocephalus. In neuroimaging tests MRI is the preferred study because of better at demonstrating multiplanar relationship, localizing middle and posterior fossa cysts, and differentatoing arachnoid cysts from other cystic lesions that may also appear hypodense in CT. Although they mostly caused obstructive hydrocephalus our operative procedure were in all cases cyst fenestrateing using open operation or endoscopyc cysternocystostomy and ventriculocystostomy. In three cases these procedures were unsucessful so we had to perform shunting operation. A residual caity may remain soon after surgical therapy, as noted on the postoperative images, bacause the rain does not expand to fill the whole space occupied by the cyst. The failure to expand merely reflects the fact that the arachnoid cyst formed early in embrionic life and was accommodated by growing brain.
