Hippocampal atrophy in normal aging and Alzheimer's disease: Postmortem analysis of four linear measures and total volume
Objective: The goal of this study has been to determine the rostrocaudal length (RCL), standard axial diameter (SAD), longitudinal diameter (LD), standard height through the band of Giacomini (SHG) and total volume of the hippocampal formation (TVHF) in normal aging and Alzheimer's disease (AD). Material and methods: Fifty-two normal and 37 AD hippocampi were neuropathologically staged and analyzed. RCL, SD and LD were obtained using precision calliper. SHG and TVHF were obtained from Nissl-stained sections using the StereoInvestigator software and Cavalieri formula corrected for shrinkage. Results: The linear regression line for TVHF during normal aging was 4.2011-0.018 * age (mm3) with r=-0.29. TVHF in age-matched controls was 2.807 +/- 0.5, but only 2.066 +/- 0.36 (mm3) in AD group, i.e. AD hippocampi were smaller by 26% (p<0.001). The atrophy correlated well with stage and duration of AD. In comparison with controls, LD was 2.4mm or 18% shorter in AD, followed by SAD (3.2mm or 14%), SHG (0.8mm or 12%) and RCL (4.1mm or 11%). Conclusion: These findings support the measurement of hippocampal atrophy in the clinical diagnosis of AD.
