Dermatitis rosaceiformis steroidica: common and unusual patients.
Topical steroids are very effective in many skin diseases because of their antiinflammatory and immunosuppresive action. However long-term use cause many side effects especially on face where dermatitis rosaceiformis steroidica is not so rare condition. For more than 20 years we follow patients with prior longer use of topical steroids and we always want to know what was the reason for it ("primary dermatosis")? Mainly it was for seborrhoeic dermatitis, rosacea and acne, but also in unrecognized dermatomycosis, cosmetic reasons and others. The majority of patients are females with facial dermatitis resembling rosacea, but more and more we register male patients and children. Dermatitis consisting of erythema, teleangiectasias, papules and pustules is developing in 3 type of distribution: perioral, centrofacial and diffuse. Perioral type is most common but diffuse and unusual cases are not rare too. In many of them mite Demodex folliculorum can be found, together with epitheloid granuloma in pathohistological analysis. Two of our female patients applied steroids for several months only on one side of the face. The other side was treated with neutral cream. The result was a heavy dermatitis only on the steroid side. So they proved an iatrogenic cause of the dermatitis. Treatment of choice in our patients are tetracyclines or isotretinoin together with local neutral compresses and creams.
