Questions from a Medical Record Database

We investigated the relationship between obesity and diabetes control in patients attending our diabetes center. Data from 1127 persons with type 1 diabetes and 10614 with type 2 were extracted from medical record database (CroDiabNET). There were significant differences in age, body mass index (BMI) and HbA1c between males and females with both types of diabetes (Mann-Whitney U, p<0.01), so further analyses were performed according to gender. There was a positive correlation between body mass index (BMI) and HbA1c in both groups (Spearman rank, p<0.01). When grouped according to treatment (no medication, oral hypoglycemic agents, insulin) type 2 patients differed in BMI, age and diabetes duration (Kruskal Wallis, p<0.01). However, there was a possitive correlation between BMI and HbA1cin all treatment groups. Mann Whitney U test revealed that male type 2 patients taking insulin vs. oral agents had the same level of diabetes control, were less obese, older and of longer diabetes duration while in females glycemic control was found to be worse. These observations are potentially helpful for our current practice analyses. So far, they confirm the close relationship of obesity and poor diabetes control. They also indicate that insulin treatment in type 2 diabetes in our practice has not been efficient. This might explain why, contrary to common opinion, insulin treatment does not seem to enhance obesity.
