Anxiety and Mioarthropathy: A Case Report

The case of 28- year- old female patient with symptoms of myoarthropathy in last year and with yellowish wear facets on teeth has been reported. The patient suffers a pain, and before also clicking in right temporomadibular joint. A case history revealed a presence of mental trauma four years ago. The increase of anxiety and the occurrence of panic attacks at predisposed patient was reported. The myofascial pain, limited mouth opening and anterior disc dislocation without reduction in right temporomadibular joint was found with manual functional analysis and confirmed with magnetic resonance imaging. As a conesquence of bruxism the interfering contacts in excursive movements of mandibule were established. The anxiety was confirmed with psychological measuring instrument State-Trait Anxiety Inventory. For initial treatment the fabrication of Michigan splint was indicated. After 2 month the improvement in clinical condition was achieved as well as mandibular function. In last few month she took irregularly prescribed anxiolytic drugs. She still feels anxious, tense and insecure. After 9 month the anterior disc dislocation without reduction and with present signs of osteoarthritis of right temporomandibular joint was established with controlling magnetic resonance imaging. Bruxism in the stomatognatic system as a psychophysiological disorder is not an isolated phenomenon and can be very significant in developement of pathologic occlusal relationships. The psychologic factors can affect a persistence of chronic paintful myoarthropathy as well as a success of initial splint treatment.
