Antiepileptic Drug Usage During Pregnancy: 10-Year Retrospective Study

In this study we analysed the characteristics of antiepileptic drugs (AEDs) use during pregnancy in correlation with possible complications of the therapy during pregnancy and delivery, and the effects of AEDs on the foetus. This retrospective study was conducted in the University Hospital Centre Rijeka, Croatia, during the ten-year period (01/01/1993 - 12/31/2002). We examined data from 151 pregnant women with epilepsy. The data were collected in a specially prepared form in which general data about the pregnant women, antiepileptic therapy and the neonatal outcome considering the use of AEDs were evaluated. In the examined period 185 children were born, and 24 miscarriages before delivery occurred in women with epilepsy. The onset of epilepsy in 90% of women occurred before the age of 20. AEDs were used in 70.2% of patients and as monotherapy in 77.4%. The most frequently used AEDs were CBZ (36.2%), VPA (29.3%), and PB (24.1%). AEDs use during pregnancy declined for 24% compared to six months before pregnancy. Folic acid was used in 25 pregnancies only, but in the year 2002 folic acid was used in 15 pregnancies (total 22). Our investigation showed that standard AEDs were the most frequently used, and no major birth defects were observed, so we might presume that the potential risk is very low. The use of folic acid in our group of pregnant women with epilepsy was improved in 2002 in comparison to the previous period.
