Intracanal Use of Dexamethasone-Neomycin Solution
The aim of the investigation was to evaluate the efficacy of dexamethasone-neomicin solution(Lek, Ljubljana, Slovenia) as intracanal medicament to prevent postinstrumentation complications. Methodology: The total of 47 patients of Endodontic clinic University of Zagreb were evaluated of incidence and severity of postinstrumentation complications such as periapical palpation and percussion sensitivity. There were two groups of patients: dexamethasone-neomycin and control, placebo group. The patients were classified according to diagnosis: 30% were diagnosed as pulp necrosis, 47% as pulp necrosis with periapical lesions (PL), 11% as required endodontic retreatment with PL and 13% as required retreatment without PL. Results: The results obtained showed postinstrumentation reactions in 8 of 47 cases (17%) as a mild periapical palpatory and percussion hypersensitivity or exudation within the root canals. In general, teeth with necrotic pulp and endodontic retreatment with periapical lesions produced the highestof postinstrumentation reactions (12%, $%). In addition, there was no significant difference between the effectiveness of intracanal medicament and placebo within24 h following complete instrumentation. So, during the second visit, we decided that root canals might be filled in most of the canals of the both groups (93%). Conclusion: Post-instrumentation complications of periapical area depend on various known and unknown factors but also on the psychophysical condition of our patients.
