Significance of a good post-endodontic restoration

Various methods for the restoring pulpless teeth have been report for more than 200 years. It is generaly belived that endodontic treatment renders teeth weaker and more subject to fracture than normal vital teeth. The reduction in tooth structure and the effect of dehydratation on the dentinal tubules are widely considered to be the main reason associated with increaded weakness. However, not only the teeth brittleness in pulpless teeth but also an apical as well as coronal microleakage has received considerable attention as a factors related to failure of treatment. Many restorative variables influence the success rate achieved in clinical practice and much lower survival rates can be expected with inadequate restoration. Under ideal conditions, a loss rate from restorative failures of 1% to 2% per year may be achievable.The unrestored tooth without a permanent restoration is a candidate for problem due to the fact that themporary restoration will not provide complete protection against occlusal forces even when the tooth is out of occlusion or splinted with an orthodontic wire. Further hard dental tissues removal should be minmized, but cuspal reduction and capping may be necessary in a distal region. The coronal restoration is retained by the core and remaining dentin and only if the core requires retention, than the canal system is used. The restoration should be designed to transmit functional loads through the tooth to the suspensory apparatus. As a permanent solution, the acid-etch composite resin is a material of choice for teeth that are otherwise lergely intact. For more extensively damaged teeth indirect restoration (onlays, crowns) provide the greatest occlusal protection.
