Use of Cast-ELISA test in the diagnosis of atopic dermatitis

Atopic dermatitis (AD) is a common, inflammatory skin disease that affect 10-20% of the population and usually represent the first manifestation of atopy in infancy. Although allergen-specific T cells and immunoglobulin E (IgE) are involved in the clinical responses to inhalant and food allergens, identification of relevant allergens is usually very difficult. In this study we investigated the diagnostic potential of cellular antigen stimulation test (CAST) in the diagnosis of hypersensitivity reactions to inhalant and food allergens in patients with atopic dermatitis. CAST-ELISA is based on the determination of sulfidoleukotrienes release by reactive cells after allergen-specific stimulation in vitro. We studied 28 patients with allergic reaction to allergens: Der p, Der f, grass polen, milk, egg and soya. We found that 69% (18/26) of patients had positive CAST to Der p, 63% (10/16) of patients to Der f, 100% (7/7) of patients to grass pollen, 24% (4/17) to milk, 39% (7/18) to egg and 40% (4/10) to soya. Our data show that CAST-ELISA is more sensitive for diagnosing hypersensitivity reaction to inhalant than to food allergens in patients with atopic dermatitis.
