Chlamyd1a pneumoniae infection as a trigger of Cogan's syndrome
Cogan's syndrome is a rare autoimmune multisystem disease. The main clinical features of typical Cogan's syndrome are vestibuloauditory dysfunction and interstitial keratitis. Cogan's syndrome is often (up to 50%) preceded by upper respiratory tract symptoms. Therefore, such an experience suggests respiratory infections as a possible trigger for the vasculitis. The only reported specific agent possibly involved in pathogenesis of the Cogan's syndrome was Clamydia pneumoniae. However, a direct link between Chlamydia species and Cogan's syndrome has not been proven. The authors present one case of atypical Cogan's syndrome with unfavorable clinical course and complete hearing loss. Positive IgA, IgM and IgG antibodies against Chlamydia pneumoniae in our patient suggest possibility of Chlamydia's role as a trigger for the vasculitis.
