Rosacea and dermatitis rosaceiformis steroidica

It is well known that rosacea can be aggravated by topical steroids. On the other hand topical steroids used for different reasons (for example seborrhoeic dermatitis or even as cosmetic preparation on healthy skin ) can cause heavy facial dermatitis resembling rosacea. This steroid induced eruption, which is not so rare facial condition, is not a variant of rosacea and can occur as an inflammatory response in any patient during or after chronic topical steroid use. For more then 25 years we follow patients with prior longer use of topical steroids on facial skin. The majority of patients are females, but more and more we register male patients and children. Dermatitis consisting of erythema, teleangiectasias, papules and pustules is developing in three type of distribution: perioral, centrofacial and diffuse. Some of our patients proved on themselves an iatrogenic cause of this condition. One 41-year-old-female patient applied various topical steroids during three years only on one side of her face. The reason for it was mild rosacea more expressed on this treated side. The other side of her face she treated only with neutral creams. The result was heavy dermatitis only on steroid side. Beside similar clinical picture with rosacea, it remains a question why both rosacea and dermatitis rosaceiformis steroidica respond very well to tetracycline treatment.
