Congenital laryngeal web in 11 year old child: diagnostic and treatment
Introduction: Congenital laryngeal web is a rare abnormality usually situated in the anterior part of glotis. This case report describes a laryngeal web in 11 year old boy unusually situated in the middle portion of glottis, as a cause of the respiratory and voice disturbances. Material & Method: The child was born after 36 weeks of gestation, as a premature newborn, with a severe respiratory distress syndrome. During the childhood he suffered from breathing problems, especially after physical efforts, and from often episodes of laryngitis and bronchitis (about 4 times a year). Permanent hoarseness was present from the very beginning. Through laryngoscopic examination was first done in the age of 11 and revealed a thin laryngeal web, about 4 mm in length, situated in the middle portion of glottis, that limitated the movements of the vocal chords. The antenior commisure was free, and that made possible the endoscopic approach in surgery. Results: The web was excised with micro scisors. After the operation the child had no respiratory disturbances, and the quality of his voice was significantly improved. The improvement of voice parameters was shown also in acoustic analysis that had been done before and after surgery. Conclusions: The unusual, middle position of the congenital web spared a patient from the complications referred to involvement of anterior commisure, and contributed greatly to excellent treatment results.
