Multifactorial analysis of cognitive outcome after keyhole approach to intracranial aneurysms

The aims of this study were to define which factors, using keyhole approach in vascular neurosurgery, contribute most to cognitive outcome. The multifactorial analysis included three groups of factors: demographic (gender, age, education), neurological (localisation and lateralisation of aneurysms, Hunt&Hess scale, type and timing of the operation) and personality traits. One hunderd and fifty patients after operation of cerebral aneurysms, in time period up to one year postoperatively, undergone the neuropsychological assessment, which included tests of intellectual functioning, thinking, memory, visual perception, visuoconstructive abilities and personality tests. They were divided in two groups according to the type of operation:large craniotomy (seventy- three patients) and keyhole approach (seventy- seven patients). Our research showed statistically better results in cognitive functions in the group of patients undergone keyhole approach aneurysm surgery. The regression analysis showed that education, age, type of operation and some personality traits were important factors of cognitive outcome. The factor analysis showed two main factors measured by psychological tests used in neuropsichological assesment- verbal and visual factor- and the type of operation seemed to be important factor of cognitive outcome for both of them. The results of this study confirmed some advantages of keyhole approach as minimally invasive technique as regards postoperative cognitive status and the quality of life.
