Common carotid artery elasticity - two dimensional versus three dimensional analysis

Background: Arterial stiffnes is associated with generation of aterosclerosis and higher risk of stroke. Aim: The aim was to validate results of three-dimensional ultrasound (3D US) analysis in evaluation of common carotid artery (CCA) stiffness. Patients and methods: Ten individuals without known risk factors except smoking, underwent CCA M-mode ultrasound, high resolution B-mode ultrasound and 3D US investigation. M and B mode slices were obtained with 10 MHz linear array transducer. 3D data sets were obtained in standardized manner, free-handed in a defined period of time (10 seconds). Recorded images were reconstructed with TomTec imaging system, and measurement was done three times, mean value was calculated. CCA stiffness was assessed by measuring CCA distensibility, expressed as luminal strain (LS) and wall shear stress (WSS). Distensibility was calculated as difference between systolic and diastolic CCA diameter over diastolic lumen diameter. Data were analyzed by Kolmogorov-Smirnov one sample test using exponential distribution. Results: Mean intima-media thickness (IMT) was 0, 53 + 0, 13 mm on B-mode US (p<0, 5) , 0, 52 + 0, 16 on M mode (p<0, 5) and 0, 61 + 0, 14 on 3D US (p<0, 5). LS CCA stiffness by M-mode was 11, 4 + 5, 72 (p<0, 5), and with 3D US 10, 6 + 3, 55 (p<0, 5). WSS CCA stiffness by M-mode was 7, 66 + 2, 61 (p<0, 5), and with 3D US 9, 01 + 4, 84 (p<0, 5, for uniform distribution). Conclusion: The results indicate a good correlation of CCA stiffness between M-mode and 3D US.
