Treting denture palatitis with miconazole gel in custom thermoformed trays-A Preliminary Study

Denture palatitis (DP) is present in 15-65% of complete or partial denture wearers. Although the aetiology is multifactorial, Candida albicans (CA) is considered as one of the most important factors in its development. The aim of this study was to evaluate the effect of miconazole gel on denture sore mouth when deliverd on a custom thermoformed trays as an overnight drug holder appliance, and to compare it with the effect of traditionally applied miconazole gel. Expected benefits of individual trays are: medicament stays longer in contact with palatal mucosa, lesser amound of drug is required, and overnight wearing is more comfortable for patients. Investigation included 22 patients diagnosed with PP. Patients were tested in order to evaluate severity of inflamation and infection. Clinical tests used were: swabs for CA cultures on Sabouraud” s agar, evaluation of inflammation untensity according to Newton and digital thermomethry. Presence and number of erosive and hyperplastic lesions, hemorrage, pseudomembranae and quantity of erythematous surfaces were inspected. Patients were divided into 2 equal groups. The experimntal group applied 2g of 2% miconazole gel (Rojazol gel, Belupo, Croatia) in custom tray for overnight wear. The control group applied 2g of the same drug 4 times a day directly in their mouth. Treatment lasted for 8 days. CA culture were performed on days 1 and 15, and all the other clinical parameters were controlled every 2 days. Data were analysed and compared between two groups. Investigation reveled that signs and symptoms of DP had healed faster in the experimental group compared to controls. Count of colonies in CA culture was also lower with mentioned method which suggest more efficient elimination of CA from the mouth. The novel method of occlusive drug delivery showed to be more effective and more convinient for the patients.
