Brucellosis re-emerging disease in Croatia
Brucella spp. are highly infectious facultative intracellular bacteria that cause zoonoses. Brucellosis is predominantly an occupational disease of those working with infected animals or their tissues, but can also infect consumers of unpasteurized milk or dairy products, as well as laboratory workers. From 1945 to 1954 Brucella infection in sheep, goats and humans has been known to be endemic in numerous parts of Istria, but since then only sporadic cases have been registred. From 1981 to 1994, a total of 45 human cases were seen exclusively in northern Croatia. No cases of brucellosis had been reported in Croatia from 1995 to 2003. This is the first report of brucellosis in Dalmatia. During 2004 two sporadic cases and one family (4 members) outbreak of brucellosis occurred in three different places of Dalmatia in association with the goats and sheep. The patients were diagnosed according to serological (standard tube agglutination test - Wright) and cultural methods. In 5 patients Brucella spp. were isolated from blood cultures performed by BacT/Alert automated blood culture testing system. The microorganism was finally identified as Brucella melitensis based on standard bacteriological techniques and by PCR technique. Identification and molecular diagnostics of Brucella spp. were performed at Croatian Veterinary Institute, Zagreb. Clinicians in Croatia should be aware that brucellosis is re-emerging and consider the diagnosis of brucellosis among febrile patients who, before onset of fever, have had direct contact with animals. Since laboratory workers are at greatest risk for acquiring brucellosis, all bacteriological diagnostic work that can produce infectious aerosol should be performed with strict attention in biological safety cabinets.
