Patient with a fixed partial denture made of an ocean shell

Introduction: Satisfaction with fixed partial denture (FPD) has a multicausal character. Some factors related to the patient are well documented: patient's personality, attitude and motivation. Aim of presentation: To present a patient who was not satisfied with her 6 previous FPDs due to material choice. Case description: She had 6 FPDs made of different materials previously (Pd-Ag + acrylic, Au alloy + acrylic, Au-Pt + isosit, NiCr ceramics, targis-vectris, acrylic removable partial denture with golden clasps). She complained that none of the materials was healthy (she considered herself a specialist in alternative medicine). She complained of headaches, progression of sight impairment, dry and burning mouth, thirsting, pain in medial eye corners and the loss of taste and coughing. Patient progress: Hydrothermal ceramic (Ducera-gold) FPD was suggested. After receiving a new FPD, she continued with all complaints and did not allow permanent fixation. She also asked if her new FPD could be made from ocean shell. After negative answer, she bought herself equipment for technicians. She also had a cast poured in a hard stone. Following 5 month she was busy drilling a sea shell and adjusting it to the cast. She asked if her "masterpiece" could be fixed permanently in her mouth. After thorough consultation about the shell composition and possible harmful effects, it was decided to fix it temporarily. Not the colour was satisfying, neither the fit nor shape. Contrary, patient was very satisfied, with no further complaints. She even reported sight improvement. She was wearing temporarily fixed FPD for one year, during which the material became porous until it was finally dissolved. She didn't want FPD of any other material and she is now "constructing" herself another FPD from a new ocean shell. The question should be raised: "What if a patient like this comes into Your office?"
