Peptic ulcer disease and its complications: surgical management
Peptic ulcer disease affects 10% of men and 4% of women at some time in their lives. Common complications of peptic ulcer disease that require surgical management are perforation, bleeding and bulbostenosis. We reviewed retrospetively histories of patients treated operatively for peptic ulcer disease complications in our hospital during 3 years period (2002-2004). 69 patients altogether were operated , 56 for perforated peptic ulcer, 10 for bleeding ulcer, 3 for bulbostenosis. The complications were as follows: we encountered dehiscence of suture line in 3 patients (4.3%). 2 of them operated for bleeding ulcer and 1 for perforation, 6 for bleeding and 1 for bulbostenosis ; 4 patients operated for perforation had intraabdominal abscess formation (5.7%). The overall mortality was 14.5% (6 patients operated for bleeding ulcer, 3 for perforation, and 1 for bulbostenosis). The mortality from peptic lcer bleeding has been high. This is because the patients admitted to hospital with bleeding ulcers were elderly with other co morbidity, delayed diagnosis and surgery.
