Preterm delivery variables - descriptive epidemiologic study
Objectives The aim of the study was to investigate epidemiologic variables influencing preterm delivery. Study Methods In the 6-year period from 1993 - 1998 all preterm deliveries from 22nd to 37th week of gestation were analyzed. Multiple pregnancies and stillbirth were excluded. Following variables were analyzed in mothers: age, matrimonial status, educational degree, occupation, smoking habits, outcomes of previous pregnancies, gestational age, modes of beginning and termination of delivery ; and in newborns: weight, Appgar score at 1st and 5th minute, need for intensive care and sub- and post-partum mortality. Results In the examined period there were 19711 (approx. 3285 per year) deliveries. After exclusion, 19397 deliveries were studied, out of which 825 (4.3 %) were preterm. Approximately half of them were the first pregnancy of the mother, and in approximately quarter of them it was the second pregnancy. Miscarriage occurred in previous pregnancy in 19 %, and abortion in 24 %. Mother's mean age was 28.6 +/- 5.9. Two hunderd and four were smokers, of them 178 were regular with up to 10 cigarettes per day. There were 81 % married mothers, 65 % were employed and 80 % had medium and high educational degree. The mean gestation of the fetus was 33.6 +/- 2.9 weeks. Deliveries started spontaneously in 42 %, and terminated spontaneously in 72 %. Twenty percent of deliveries terminated with caesarean section. The most frequent Appgar scores in the 1st minute were 8 - 10 (50 %), as well as in the 5th minute (66 %). Intensive care was needed in 44 % of the newborns. During the delivery 8 (1 %) fetuses died and during the first week after delivery 93 (11.3 %) newborns died. Conclusions Preterm delivery was more frequent amongst married, employed, medium/high educated and moderate smokers.
