Cutaneous odontogenic sinus tract: a case report

A sinus tract is defined as a passage of puss from an abscess cavity to an external environment through a tissue membrane such as oral mucosa or skin - depending on the anatomical location of the tooth apex. Odontogenic cutaneous sinus tracts of the face region are rare and may present a diagnostic problem. The differential diagnosis should include bacterial and fungal infection, local skin infection, chronic tuberculosis lesion, developmental defects, traumatic lesions, etc. Radiographic findings are also important for the diagnosis. A clinical examination of a 47-year-old female revealed a nodulo - cutaneous lesion on the right cheek 1 cm above the inferior border of the mandible. Previous treatment was performed by dermatologists and included surgical and chemical burn therapy 3 to 4 times. The nodule reappeared each time after treatment. After examination by oral surgeon the disorder was recognized as a dental problem and the patient was referred to Department of dental pathology. Radiographic examination showed radiolucent periapical lesion associated with root apex of right mandibular first molar. A cord-like tissue formation was detected by palpation of buccal mucosa in the area of the suspect tooth. Conservative endodontic non-surgical root-canal treatment was performed and sinus tract healed completely.
