Use of CAD/CAM in manufacturing of wedged orthopaedic insoles for treatment of gonarthrosis
Gonarthrosis is common degenerative joint disease. Etiology address to biomechanical stresses, biomechanical changes, genetic and nutritional factors. Patients often have knee pain, difficulty walking and reduction of daily activities. The recommended approach to the medical management of gonarthrosis includes drug therapy and non-pharmacologic modalities, which include wedged insoles. Lateral or medial wedged orthopedic insoles through decreasing of pressure in medial or lateral compartment of the knee can lead to significant pain relief. We use Insole Modeler CAD/CAM software with Supination and Pronation functions for fast generating of wedges at the insole surface. Placing of the wedge is easy and accurate, and SW has option for undoing changes. CAD/CAM system comprise several modules: module for collection of biomechanical data, software for procession, analyze and design of orthotic surface which is then converted into CAM machine program, that runs milling machine.CAD/CAM shows multiple advantages when compared with classic (manual) approach of insoles manufacture. Use of wedged orthopedic insoles for treatment of knee gonarthrosis with pain in medial or lateral knee compartment in our practice so far is comparable to results in recent literature ; all our patients after the application of individual wedged orthopaedic insoles were subjectively better with improvement of daily activities.
