A case of HPV-16 related Bowenoid papulosis of the penis successfully treated with topical imiquimod cream

Bowenoid papulosis (BP) is a rare dermatosis of ano-genital region with variable clinical feature: multiple, generally small, round papules, isolated or confluent, with smooth or papillomatous surface, sometimes with desquamation. The colour is variable from rose, reddish-purple to brown. BP represents a form of squamous cell carcinoma in situ, mostly associated with infection with high-risk oncogenic human papillomavirus (HPV) types 16, 18, 31, 33, and others. The risk of progression to invasive cancer is low. Treatment usually involves locally destructive or ablative procedures. The condition usually shows a good response to immuno-modulatory treatment with imiquimod, a topical immune response modifier with indirect antiviral and antitumor effects through the stimulation of local cytokine production and cell-mediated immune response. Herein, we describe a 22-year-old male patient who had multiple reddish-papules like BP of the penile shaft. Biopsy specimen showed the typical histological pattern of squamous cell carcinoma in situ. The presence of HPV type 16 was determined by means of polymerase chain reaction (PCR) in DNA extracted from a biopsy sample. We report a well-tolerated, successful clearance of this BP of the penis upon treatment with topical 5% imiquimod cream, once every other day over the period of two months. Redarding the oncogenic potential of high-risk HPV infections, irrespective of the treatment used, all patients with BP and their sexual partners should be included in long-term follow-up and regular examination for possible malignant outcome.
