The monitoring of patterns of antimicrobial drug use is very important because of development of resistance, growing costs and potential adverse drug reactions. Drug use review programs are commonly accepted as a means to foster appropriate use and may assist in controlling expenditures. Most programs focus on qualitative evaluation ; however, quantitative review provides valuable data on use patterns and may often be a motivating factor for a qualitative study. Reliable data on drug consumption should ideally be based on the treatment of individual patients. However, as such data are seldom readily available, drug utilization studies are performed to analyze data obtained over a certain period of time or to evaluate the pattern of use and the effects of interventions. The well-established and widely accepted method that facilitates monitoring of drug use (including antimicrobials) is the ATC/DDD methodology recommended by the WHO. It is independent of sales prices, package sizes and actually prescribed daily dose. We present the antimicrobial drug use evaluation in Rijeka by using this methodology. The University Hospital Centre Rijeka is the second largest hospital in Croatia serving the population of County of Primorje and Gorski Kotar. The antimicrobial drug consumption is presented in DDD/100 bed days. The utilization data covers the period from 1990 -2004. The data on outpatient antimicrobial drug use is expressed in DDD/1000 inhabitants daily and it covers the data from the last 5 years. The total use as well as the pattern of antimicrobial drug use in Rijeka varied over time. There are number of factors that may play a role in antibiotics prescribing. Different culture and therapy traditions, patient’ s pressure, local formularies and policies, and resistance patterns are only some of them. We will describe trends in antimicrobial drug use and try to identify possible factors that influenced changes in antimicrobial prescribing patterns in Rijeka.
