MINIMALLY INVASIVE TECHNIQUES IN NEUROSURGERY

Minimally invasive and less traumatic concept in neurosurgery became imperative in past years. Better diagnostic facilities, developement of instrumentation and microsurgical techniques gives the oportunity for more precise targeting the patological processes placed intracranially. The era of modern neurosurgery begin with introduction of operative microscope during operations. Further improvement in illumination and visualisation was possible with introduction of endoscopic techniques. Today laser has become a standard tool in modern minimally invasive microneurosurgery and it is being increasingly used in functional neurosurgery as well. Lasers have found their full and extensive application for their capacity of acting on pathological tissues without direct contact, through relatively small craniotomy, and in deep hardly accessible regions. Neuronavigation techniques tracks the position of a specialized instrument in relation to surgical anatomy and identifies this position on diagnostic images of the patient. It helps surgeon to make precise trajectory to the target with limited exposure of healthy tissue above the target. Keyhole concept in neurosurgery has been increasingly used in the past few years. We used it in our everyday practice for treatment of different vascular and tumor pathology. From 1996. almost 800 patients with diferent intracranial pathology were operated through keyhole approach. Minimally invasive procedures in neurosurgery have distinct advantages over standard techniques like better functional outcome, less exposure of intact brain, and shorter duration of the procedures. Furthermore loss of blood during operations was decreased in past period during the operations performed on the basis of minimally invasive principles.
