Preoperative and postoperative diagnostics for cochlear implantation

The range of cochlear implantation outcome can be wide, depending on many factors, therefore detailed multidisciplinary diagnostic treatment is necessary with the decision based on a team work. Diagnostics for cochlear implantation includes preoperative and postoperative procedures. Preoperative diagnostics has two objectives: 1. to precisely evaluate the level of peripheral hearing loss and thus the indication for CI, 2. to evaluate the existence of favourable but also non favourable factors for the procedure so that surgical and rehabilitation treatments could be adjusted accordingly and that patients and care-givers could be given as much as possible realistic outcome projections. Diagnostic protocol consists of assessment of all levels of hearing and vestibular sense organ, neurological and psychological assessments, physiatrist, laryngologist, hearing pathology, speech pathology examinations and radiological evaluation. The aim of postoperative diagnostics is to monitor all those postoperative factors that influence the results of CI, guiding the rehabilitation course of each individual CI user. Systematic and thorough postoperative follow up is also necessary in cases of slower or delayed progress, when objective data can beside encouraging and guiding the rehabilitation process also help explaining cases of modest rehabilitation outcomes. It is equally important to compare rehabilitation outcome with preoperative findings, continually verifying correctness of preoperative criteria and estimation. The protocol changes in accordance with new knowledge and technological development. The paper presents our preoperative diagnostics protocol, postoperative assessments program, expected listening and speech development progress in children with isolated hearing impairment and diagnostics algorithm applied in cases of slower listening and speech development of CI children.
