Early diagnostic of hearing impairment. Patient selection for cochlear implantation

Cochlear implantation contributed greatly in deafness problem-solving, opening new possibilities in rehabilitation. In spite of big differences among the results achieved by patients, investigation and follow-up so far showed undoubted regularity- younger the age at surgery, better the results. Evidences on significance of early neural development support the idea of early detection and intervention with the aim of reaching maximal level of hearing development and communication abilities. Diagnostic of very small children is much more demanding than similar procedures in adults. Tests results sometimes are not unambiguous and need repetition. Implementation of universal newborn hearing screening by OAE in Croatin hospitals enables early identification of hearing impairment. Confirmation by automatic ABR is the second step. To set the indication for cochlear implantation the battery of tests that show morphology of the ear, condition of neural pathways and psychomotor development of the child should be done. Time between establishing the diagnosis and possible operation should be used in rehabilitation for stimulation of central hearing pathways in this period of high neuroplasticity and also to follow up hearing development assisted by conventional hearing aids. This provides valuable data about possible additional disturbances that could influence rehbilitation that were not recognised by the tests previously done.
