Influence of Smoking and Parity on Serum Marker Levels in Mid-Trimester Screening for Down's Syndrome
OBJECTIVE: To evaluate the influence of smoking habits and number of births on maternal serum alpha-fetoprotein (MS-AFP) and free beta chorionic gonadotropin (F-beta hCG) as markers in mid-trimester screening for Down's syndrome. STUDY DESIGN: A total of 1740 unaffected singleton pregnancies consisted of 1379 non-smoking and 361 smoking mothers. All pregnancies ended with a birth of normal child. Preterm deliveries and abortions were excluded from the study. The mean maternal age was 32, 61+/-5, 09 years (mean+/-SD) in non-smokers and 32, 83+/-5, 49 in smokers, respectively. Each group was separated in relation to parity and effect on serum concentrations was evaluated. Concentrations were expressed as multiples of median (MoM) at the relevant gestational age fot unaffected pregnancy and adjusted for maternal weight. RESULTS: We found significantly reduced serum level of F-beta hCG in smoking mothers in relation to non-smoking ones (0, 97+/-0, 18 versus 1, 13+/-0, 3 p<0, 05). MS-AFP showed no significant difference. In regard to parity, F-beta hCG levels were significantly affected with the number of births (p<0, 05) ; the effect not found with MS-AFP. Researching smokers and non-smokers separately, we noticed gradual decreasing of F-beta hCG in serum with multiparity. The statistical difference was evident in non-smokers (p<0, 05) and even more in smoking mothers (p<0, 01). CONCLUSION: According to our results, the influence of smoking and parity, particularly on f-beta hCG as biochemical marker for Down's syndrome, is significant. This observation may be worth in consideration of its effect on screening performance.
