Diffuse symmetrical lipomas of the neck (Madelung's neck) and epilepsy
Madelung's neck is rarely associated with other pathologic disorders in the function of the hepatobiliary, endocrinologic and nervous system. Reports of cases with this deformity and epilepsy are even less frequent. Presentation is made of a 28-year-old man, admitted for tonic-clonic status epilepticus. Physical examination revealed circumscribed lipomatosis of the neck. Interictal EEG finding was diffusely dysrhytmic, whereas karyotype analysis showed a picture of chromosomal mosaicism, 46x/47xy+1. Repetitive generalized tonic-clonic seizures would not subside with any of the antiepileptic therapy regimens applied. The patient died at the age of 30, with a picture of cerebral hemorrhage. The patient's daughter has been manifesting epileptic seizures of a partial complex type with secondary generalization since the age of 16. Her EEG findings showed focal alterations, frontally on the left. Magnetic resonance of the brain revealed an arachnoid cyst located frontopolarly parasagittally on the left side. The patient's karyotype showed normal female constitution, 46xx.
