Pap Smear Adequacy: Is the Assessing Criterion Including Endocervical Cells Really Valid?

The significance of endocervical cylindrical cells (EC) as a criterion of sample adequacy has been established on 1000 testees by comparing VCE smears with or without EC in relation to the number of positive and false-negative findings, sensitivity, specificity and predictive values as well as by comparing the negative findings without EC with control smears aiming at subsequent revealing of overlooked lesions. A significantly higher number of discovered abnormalities in smears with EC (107/536 in relation to 49/464) and a higher sensitivity (95% in relation to 80%) as well as positive predictive value (93% in relation to 90%) of those samples favour the hypothesis that the presence of EC is connected with the discovery of abnormal cells in smears. The fact, however, that the number of false-negative cytologicalal findings and the number of newly discovered abnormal findings did not significantly differ in terms of the presence /absence of EC (P>0, 05), favours the hypothesis that the smears not containing endocervical cylindrical cells do not imply a higher risk of predicted abnormalities, so it is not necessary to repeat them earlier.
