Castelman's disease - Report of case
Objective: Castelman's disease (CD), as a rare disease of children, is often difficult to diagnose cytologically as well as histologically. We report here a case diagnosed postoperatively by pathological criteria and immunohistochemical staining. Case report: A 5-year-old girl with an enlarged lymph node in the neck, subangulary right, slowly had increased in size for the previous three years was examined.Ultrasound-quided fine needle aspiration of a hypoechogenic zone (43x20 mm) was performed with 21-gauge needle. Cytological diagnosis of non-Hodgkin's lymphoma (NHL) was established, and the patient immediately underwent surgery. Postoperative pathohistological examination (including CD 35)revealed Castelman's disease (hyaline-vascular type).CD is a rare atypical lymphoproliferative disease, morphologically and clinically heterogeneous, yet strongly connected to the risk of developing malignant lymphoma. On the basis of the cytological result, urgent surgical treatment was suggested. A final cytological diagnosis could be made only upon the final pathohistological diagnosis. Conclusion ; More experience is required in evaluating cytological findings of lymph node hyperplasia in children in order to be able to establish a proper diagnosis before surgery and to inprove diagnostic accuracy.
