Acute renal failure due to cardiogenic shock in older patients

Eleven patients aged 66 to 81 years with state of cardiogenic shock caused by myocardial infarction are presented. In all patients the following parameters of renal lesion were analysed after the restoration of diuresis: sodium in urine, osmolality quotient of plasma, osmolality of urine, osmolality quotient of urine and plasma, the renal failure index and the fractional excretion of filtered sodium. In 7 patients natriuresis indicated acute functional oliguria, in 3 patients acute renal faiulure and in 5 cases indicated bordeline values. In 7 patients the values of urine osmolality were in the range of functional oliguria, while in 4 patients had bordeline values beetwen functional oliguria and acute renal failure, as well as the osmolality quotient in urine and plasma. The values of the renal failure index in all patients were lower than 3.0 (in liwer than 1.0) indicating functional oliguria, as well as the functional excretion of filtered sodium. Acute renal due to cardiogenic shock patients is functional and it is rarely renal.
