Head injury as provocative factor for epilepsy in the elderly
The diagnosis of traumatic epilepsy depends on the severity of head injury, duration of initial unconsciousness and neurological findings. We have analised 77 epileptic patients (18 female and 59 male) with head injury in their history. The youngest was 19 and the oldest 69. Female (33%) were mostly in the group from 19-29 years, and male (37%) in group from 40-49 years. 86.7% patients were workers, 12% retired-person, one nurse (1, 3%) and one house-wife (1, 3%). Closed head injures (including intracranial haemorrhages) were evaluated in 48 (62%) patients, open head injuris in 29 (38%). Onset of traumatic epilepsy was the most frequent during first year after brain damage, in 45% of our cases, in second year in 17%, rare in third year 2, 6%, and relatively frequent after four year - 35.1%. According to ILAE definitions 52 (67%) patients had generalised tonic-clonic seizures and 25 (32%) had partial seisures. EEG was abnormal in 95% on the total, mostly with focal changes (49%). Respecting all modern principles of treatment of epilepsy 37% our epileptic patients had poor seizures control.
