Relationship between the position of the condyle and anterior displacement of temporomandibular joint disc

The objective of the investigation was to determine the relationship between the position of the condyle head in the glenoid fossa and anterior disc displacement of temporomandibular joint. The investigation was conducted on 40 patients with clinical symptoms of disc displacement of temporomandibular joint. The temporomandibular joints were recorded bilaterally by magnetic resonance imaging and as a result partial and complete disc displacements with reduction, complete displacement without reduction as well as a group of joints with physiologic disc position were diagnosed. Two joints in female patients with asymptomatic displacement of the articular disc were excluded. Position of the condyle and disc was calculated using the Kurita et al. method on the parasagital view of the joint. Methods of nonparametric statistics were used (Kruskal-Wallis test) and in data analysis the left and the right temporomandibular joints of a single person were presented as two entities. A statistically significant difference between the position of the disc in joints with physiologic position and joints with disc displacement was determined (p<0.01), but no difference in condyle positions depending on the existence of disc displacement was found (p>0.05). The measured values of the groups of joints in patients with individual diagnoses of disc displacement were compared. No significant difference in either the disc position or in condyle position (p>0.05) was determined in individual diagnoses of articular disc displacement. In conclusion, the connection of the dorsocranial condyle head position with anterior disc displacement of temporomandibular joint was not determined.
