Transcranial Doppler Sonography Of Posterior Cerebral Artery In Patients With Vertebral Artery Occlusion

One fourth of all ischemic strokes occur in posterior circulation. One third ischemic strokes are caused by large artery disease ; in some extent vertebral artery occlusion is cause of stroke in posterior circulation. Transcranial Doppler sonography is non-invasive method, excellent temporal resolution for real time investigation of cerebral hemodynamcs. In this study mean blood flow velocities (MBFV) in posterior cerebral artery (PCA) were measured before and during visual stimulation. During eyes closed MBFV showed no significant difference between groups. During white light visual stimulation statistically significant increase of MBFV in PCA in all groups occurred. In control group increase was 22, 85± 20, 9% for right and 20, 67± 15, 31% for the left PCA. Right ACP in all patients with VA occlusion showed increase of MBFV of 12, 30± 16, 46% and left 11, 89± 13, 08%. Patients with isolated VA occlusion showed increase of 15, 15± 15, 98% in the right and 12, 57± 15, 52% in the left PCA. Response in group of patients with VA occlusion and carotid disease was low: 8, 57± 14, 4%, 10, 07± 14, 56% for right and left PCA, respectively. In PCA ipsilateral to VA occlusion response was lower then on the contralateral side: 9.34% and 13.53%, respectively. In patients with VA occlusion exhaustion of cerebral vasoreactivity was present. Visually evoked functional TCD testing is non-invasive, precise method for estimation of vasoreactivity in posterior circulation.
