Neurological, physiatric, psychological and psychiatric diagnostics in children with cochlear implant
Cochlear implant is the most technologically sophisticated hearing aid that provides optimal development of hearing and speech. If the peripheral input is enabled the hearing is not achieved yet, because the central mechanisms have been inactive and have to « ; learn» ; how to transmit the acoustic information, how to clean and structure the signal. Rehabilitation of hearing and speech has to do on central level what technology did on the peripheral one. Diagnostic is functional, topologically orientated to the mature level of spacioceptive functions. After one year of cochlear implantation and rehabilitation the subjects show significant improvement in hearing level, speech perception and production. The first results indicate the significant role of hemisphere dominance on hearing achievement in cochlear-implanted children. The peripheral hearing function influences the central listening abilities and lateralization in auditory processing. Evoked potentials testing with cortical cartography shows as a useful diagnostic method for visualization of functional changes in spacioception. The influence of vestibular and motoric function in hearing and speech abilities due to maturation process of CNS can be measured by stabilometry testings. Psychological evaluation results show as a significant predictor of the rehabilitation achievement in cochlear implanted child. According to the age of cochlear implant user, the battery of tests are being performed to the better visualization of cognitive, psychomotor, visuomotor and language skills. Psychiatric diagnostic and therapy approach is supportive orientated to the active role of cochlear implant user in rehabilitation process.
