Infertility and immunoglobulins therapy

Objective: Registration complication, side effects and results of intravenous immunoglobulins therapy (IVIG) in pregnancies with history of recurrent spontaneous abortion. Study design: A prospective study in 25 women with habitual abortions in the first trimester. Indications for including in the study were; excluding anatomical, endocrinological, genetic, microbiological and other environmental cause of infertility in couples sharing HLA-D antigens. Results: Out of 25, sixteen pregnancies have finished with healthy liveborn babies. One baby was born in 28th week of gestation by caesarean section. This baby died three hours later. Eight have miscarried. That means the possible successful rate of treatment would be 68%. Four miscarriages were in the first trimester of gestation without known reasons. One pregnancy finished in first trimester due to chromosome mosaicism of product of concepction, another one due to submucosal mioma. Two abortions were in second trimester due to partial septate uterus and another due to cervical incompetence. Last three mentioned cases were results of wrong selection or wrong following control. If we exclude these cases and known chromosomal reasons of abortion, successful rate of therapy with IVIG will be 80,95%. Cervical cerclages and caesarean sections were in normla range. Control laboratory and clinical examinations of the successful pregnacies were always normal. The side effects of treatment with IVIG occurred only during first infusion, but minor, in ten cases. Conclusions: Real value of immunoglobulins therapy will be possible evaluated after meta analysis of more number of treated cases.
