The Siginificance and Severity of Drug-Drug Interactions among all Reported Adverse Drug Events (ADEs) in Croatia

The prevalence of drug-drug interactions (DDIs) among all reported adverse drug events (ADE) to national Agency for medicinal products and medical devices (AMPMD) was investigated. Exploration of significance and severity of drug-drug interactions is in purpose of development of pharmacists’ interventions in preventing further occurrence of drug related problems. This study is set up as both, retrospective and prospective analysis. A retrospective study was conducted on a sample of 498 reports of ADEs reported to national Agency for Medicinal Products and Medical Devices in a period from March to December 2005. A prospective observational study will be carried out from January 2007 until December 2010 where all ADEs due to DDIs reported to Agency will be analysed in the same manner. Identification and confirmation of ADEs due to DDIs was carried out by micromedix program. Results of the analysis showed that among all ADE reports, 32.5% (162) refer to vaccines, while 67.5% (336) refer to all other drugs. Findings showed that 25% of all ADEs are serious. Out of all ADEs 7% were due to DDIs out of which 50% are classified as serious. These findings indicate higher prevalence of drug-drug interactions in Croatia in comparison with world’ s average data (5%). The majority of drugs which caused DDIs belong to N and C ATC group of drugs. The most DDI reports are referred to cyclosporine in concomitant therapy with either fluvastatine or methylprednisolone which resulted in hospitalization and withdrawal of therapy. The association and influence of principal demographic variables (age, gender, number of medicines etc.) on the frequency/prevalence of drug interactions was also explored. Following the results of the retrospective and prospective analysis, research methods for specific pharmacists’ interventions will be developed and its influence measured with the aim of reducing drug related problems.
