Frequencies of positive celiac disease serologic test results in at-risk groups of patients in Croatia

Aim . Aim of this study was to determine frequencies of positive celiac serologic tests: antigliadin (AGA) IgA, IgG and anti-tissue transglutaminase IgA (tTG IgA) antibodies in at-risk groups of patients. Methods. During the 4-year period, 1184 consecutive patients(median age 11, interquartile range: 5-16 ; 41.8% males, 58.2% females) with suspected celiac disease (CD) were tested for AGA IgA, AGA IgG and tTG IgA with ELISA. For 77 patients who consented to undergo jejunal biopsy CD was confirmed. Results. In total of 1184 patients positive results were as follows: AGA IgA 151 (12.8%), AGA IgG 378 (31.9%) and tTG IgA 125 (10.6%). All tests were positive in 70 (5.9%), two tests in 72 (6.1%) and one test in 299 ( 25.3%) patients. Patients with biopsy confi rmed CD had following positive results: AGA IgA 55/77, AGA IgG 73/77 and tTG IgA 63/77, with positive all tests, two tests or one test in 49/77, 18/77 and 8/77, respectively. IgA deficiency was found in 7 of all biopsy confirmed patients. The most frequent presenting symptom for biopsy confirmed CD patients was malabsorption (62.2%) and within all tested group diabetes mellitus (18.5%). Conclusions. The most frequent positive test result found in entire included group of patients and in those with confirmed celiac disease was AGA IgG. The mo st frequent presenting symptom for CD serologic testing was diabetes mellitus, but in biopsy confirmed disease malabsorp tion was the most frequent disorder.
