Arthroscopic extra-articular knot technique for recurrent anterior shoulder instability

Aim of the study is to quantify functional outcome following arthroscopic Bankart repair using extra-articular knot technique for recurrent anterior shoulder instability. Material and Methods Between 1998 and 2003 fifty-eight operations using extra- articular knot technique for recurrent anterior shoulder instability were performed. There were 46 men (47 shoulders) and 11 women with average age of 27 years (14 to 65). An average follow up was 48 months (24 to 89). They had on average 6 to 10 shoulder dislocations prior to operation ; forty- nine were result of the trauma. There was no patients selection for arthroscopy regarding age, number of dislocations and type of activity. On average three suture anchors were used and minimally one extra-articular knot applied. For evaluation history was taken, clinical examination done and ASES and Constant score obtained. Results Four patients (6.9 %) out of fifty-eight had redislocation after surgery. Two of them were operated by open procedure. An average Constant score was 92 points (from 78 to 100 points), average ASES being 90 (from 63 to 100 points). An average loss of active external rotation with an arm in adduction compared to non-operated arm was 7 degrees and when arm was positioned in 90o abduction average loss was 4 degrees. Discussion / Conclusion Arthroscopic Bankart repair with extra-articular knot for anterior shoulder instability gives good clinical and functional outcome and results are comparable to open Bankart repair.
