Prognostic value of echonographical parameters in acute pancreatitis

Aim: To determine prognostic value of echosonographical parameters in acute pancreatitis. Methods and patients: 130 patients from 18 till 82 years of age suffering from acute pancreatitis and admitted to intensive care unit were included in the study. In all patients ultrasound measurements of pancreas were determined, three times during their hospitalisation: 1st time immediately after hospitalisation, 2nd time after 48 hours of hospitalisation and 3rd time on the fifth day of hospitalisation. Biochemical parameters were: amylase in urine and sera, AST, ALT, GGT, ALP, glucose, Hgb, RBC, WBC, ER, pCO2 and base deficit. Patients were divided by Ranson’s criteria, sex and etiology of pancreatitis (biliary, alcocholism). For each of these parameters there are determinations of single and group correlations with severity of disease and accompanning complications. Results: Growth dimension of head of pancreas measured between second and fifth day of hospital stay is prognostically ominous. A significant predictor of a severe form of acute pancreatitis is strengthened by biochemical tests: hyperglycemia, hypoxia, hyperamylasuria, and high level of AST. It has also been found that alcocholism together with acute pancreatitis in middle aged men represents bad prognostical sign . 46 patients from 130 of them suffered severe form of pancreatitis, 45 of them with alcocholic pancreatitis. Conclusion: Severe form of acute pancreatitis is possible to discover on time by a subset of echosonographical, biochemical and clinical parameters.
