The value of current emergency surgical treatment of war head injuries – a higher survival rate and reduced proportion of post-traumatic epilepsies

The experience of neurosurgical services acquired during the war and post-war period justifies the opinion according to which patients with missile wounds to the head should be rapidly transported to the neurosurgical team, since their survival and future quality of life can be significantly improved by emergency surgery. At the same time, a fully elaborated and uniform surgical procedure is the best prevention of post-traumatic epilepsy. At present, the basic principles include the earliest possible resuscitation and antiedematous therapy, fast transportation to an institution with emergency neurotraumatology service, diagnostic orientation and procedure planning according to emergency CT, and emergency operation for the best possible removal of foreign bodies introduced by the missile, haemostasis and decompression to minimize secondary brain lesions, maintenance of physiologic compartments as possible (with subsequent reconstructive surgery), and earliest possible introduction of antiepileptic drugs prophylaxis. Our own experience and results are presented.
